Paradontological Treatment

Initial periodontal therapy implies procedures of the mechanical removal of the cause of periodontal inflammatory diseases. Gingivitis (inflammation of the soft tissue) and periodontitis (inflammation of the supporting teeth structures) are mainly caused by bacteria, and therefore the goal of therapy is to minimize their number. Bacteria are found in the so-called biofilm, deposits found on the surface of the tooth root, which protects them from external influences.

Protected by biofilm, bacteria can not be eliminated by the use of antibiotics, which is a very common misconception, but the biofilm needs to be “broken” and removed by mechanical procedures. This procedure is done by hand instruments, so-called curettes and mechanical, sound and ultrasonic instruments. Depending on the progression of the disease, the therapy is performed in several visits. The deposits are removed above (“surface cleaning”) and under the gingiva (“deep cleansing”).

The procedure is extremely painless because mechanical therapy under the gingiva is performed under local anesthesia.

This type of therapy is also referred to as closed, conservative therapy because cleaning is done without moving the gingiva, therefore without a direct insight into the pocket and the surface of the root.

Sometimes the results of conservative treatment are not entirely satisfactory, for example when after unsurgical therapy remain deeper pockets. Then it continues with corrective, surgical therapy.

The precondition for successful surgical therapy is good oral hygiene so that the procedure can be performed with less bleeding, and the healing after the procedure can be carried out smoothly. Surgical periodontal therapy, considering its goals, can be divided into resective, regenerative and mucogingival therapy.
